HELICAL SWEEPS- DON'T ALWAYS SEEM TO WORK.

A few questions on the Helical Sweep, I'm putting threads on an extruded circle (making a bolt).  I can get it to work, but only after lots of frustration. 

1. When you create the Line Profile, do you have to draw the line exactly along the side of the bolt where the thread will be?   (I watched a bottle cap video tutorial on YouTube, which was very good by (Huu Huynh) but his Profile Line was about 10 units inside where he drew the triangle and it worked fine.)

 

2. Do you have to make your edit sketch along the Line Profile or can it be completely outside of it (related to question one)

 

3. When you draw the triangle (edit sketch for the threading), it seems like you have to place it a bit outside the bolt line for it to work.  Doesn't this ruin the accuracy of the pitch settings?  

 

4. Finally, when you get the FAILED TO REGENERATE error,  is there anyway to find some clues about why it failed?

 

"  Unattached Helical Sweep detected???? ""

 

If anyone can shed some light, greatly appreciated.  I've attached a picture of the scenario I'm trying to describe.
